Private Health Insurance in Portugal

Portugal has a good public healthcare system (SNS), but many expats also take out private health insurance for faster access, English-speaking doctors, and coverage during the waiting period for SNS registration. Some visa types require health insurance as a condition.

Determine if you need it

Required for: D7/D8 visa applications (private insurance or proof of SNS access), Golden Visa, and any period before SNS registration is complete. Optional but common for: faster specialist access, private hospitals, dental and optical coverage not well covered by SNS.

Understand the main providers

Major insurers: Fidelidade, Allianz, Multicare (from Millennium BCP), Medis, AdvanceCare. International options: Cigna Global, Allianz Care, Bupa Global. Local plans are much cheaper but may have Portuguese-only customer service.

Compare plans and costs

Individual plans: 30-150 EUR/month depending on age, coverage, and co-pays. Family plans available. Key things to compare: hospital network, co-pay amounts, dental inclusion, pre-existing condition waiting periods, and cancellation terms.

Apply and get your policy

Most insurers offer online quotes. You'll fill in a health questionnaire. Pre-existing conditions may be excluded or have waiting periods (typically 6-12 months). Coverage usually starts within a few days of payment.

Use alongside SNS

Most expats use a dual system: SNS for routine care and emergencies (free or very low cost), private insurance for specialists, diagnostics, and when you want faster access. Private hospitals: Hospital da Luz, CUF, Hospital Lusíadas are the main chains.

Watch out

  • Visa applications require insurance valid from day one in Portugal — arrange before arrival
  • Pre-existing conditions often have waiting periods or exclusions. Read the fine print
  • Private insurance does NOT replace SNS registration. Register for both
  • Dental and optical coverage is usually an add-on with extra cost
